By accessing and using the Jevantrad Network website and scheduling platform, you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, please do not use our website or services. These terms apply to all visitors, patients, and users of our online scheduling system.
2. Medical Services Disclaimer
The information provided on this website is for general informational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of Dr. Ddamulira or another qualified healthcare provider with any questions you may have regarding a medical condition. 3. Appointment Scheduling
Our online scheduling system allows you to request appointments with Dr. Marion Ddamulira, DNP. Submitting a booking request does not guarantee an appointment. Appointments are confirmed based on availability and eligibility. We reserve the right to reschedule or cancel appointments when necessary and will make every effort to notify you in advance. Home visit eligibility is determined on a case-by-case basis and is not guaranteed for all patients.
4. Telehealth Services
Telehealth visits are conducted through secure video or phone connections. By scheduling a telehealth appointment, you acknowledge that: you have access to the necessary technology (smartphone, tablet, or computer with a camera and internet connection); telehealth may not be appropriate for all medical conditions; Dr. Ddamulira may determine that an in-person visit is necessary after the telehealth consultation begins; and you are responsible for ensuring a private, quiet location for your virtual visit.
5. Patient Responsibilities
As a patient of Jevantrad Network, you agree to: provide accurate and complete medical and insurance information; arrive on time for scheduled appointments or join telehealth visits promptly; provide at least 24 hours notice for cancellations or rescheduling; treat Dr. Ddamulira and all staff with respect; and follow the care plan and treatment recommendations discussed during your visit.
6. Insurance and Payment
Patients are responsible for understanding their insurance coverage, including co-pays, deductibles, and any out-of-pocket costs. While we accept most major insurance plans, coverage varies by plan and individual policy. We recommend verifying your coverage with your insurance provider before your visit. For self-pay patients, payment is due at the time of service unless other arrangements have been made in advance.
7. Cancellation Policy
We require at least 24 hours notice for appointment cancellations or rescheduling. Repeated no-shows or late cancellations may result in a fee and may affect your ability to schedule future appointments. We understand that emergencies happen, so please contact us as soon as possible if you need to cancel or reschedule.
